Visitors to the Biochemistry Department

FORMS TO BE COMPLETED BY VISITORS TO THE DEPARTMENT OF BIOCHEMISTRY

FOR ALL VISITORS- All external visitors will be required to show their passport/visa to verify their status in the UK.

Visitors Checklist: A Guide To Visa And Immigration Status Check Requirements

External Visitors here for up to two weeks

Please obtain approval for the visit with the appropriate Laboratory Head and advise Administration accordingly. The visitor will also be requested to show their passport. If they are from outside the EU, please check with Administation that they have the relevant paperwork before they enter the UK.

1.    Health and Safety form - to be completed/signed by both the sponsor/host and visitor.  The form must be taken to Steve Curnock (Acting Safety Officer) on the day of arrival so that he can give the visitor a Health and Safety induction, please make an appointment with Steve in advance – no work of any kind must be carried out in the department until after the visitor has undergone induction. 
No access or ID cards will be issued – Visitors will need to be escorted into buildings as required by the departmental host.

External Visitors here for between two weeks - 1 month

1.    External Visitors Form – required in advance of the visit and to be approved by the Associate Head of Department for the appropriate Research Laboratory before plans for the visit are finalised.

2.    Health and Safety form - to be completed/signed by both the sponsor/host and visitor.  The form must be taken to Steve Curnock (Acting Safety Officer) on the day of arrival so that he can give the visitor a health and Safety induction, please make an appointment with Steve in advance – no work of any kind must be carried out in the department until after the visitor has undergone induction.

3.    Key and Proximity card deposit_receipt form (acts as a receipt for refundable £10 deposit). Deposits to be paid to Admin before cards are issued.

External Visitors here for longer than 1 month

1.    External Visitors Form – required in advance of the visit and to be approved by the Associate Head of Department for the appropriate Research Laboratory before plans for the visit are finalised.

2.    Health and Safety form - to be completed/signed by both the sponsor/host and visitor.  The form must be taken to Steve Curnock (Acting Safety Officer) on the day of arrival so that he can give the visitor a health and safety induction, please make an appointment with Steve in advance – no work of any kind must be carried out in the department until the visitor has undergone Health and Safety induction.

3.    University ID card application form – gives access to University e-mail.

4.    Key and Proximity card deposit_receipt form (acts as a receipt for refundable £10 deposit). Deposits to be paid to Admin before cards are issued.

Visitors/Collaborators/Students from other University Departments or Colleges

1.    Visitors from other University Departments and Colleges form – required in advance of the visit and to be approved by the departmental host.

2.    Health and Safety form – to be completed by both the student and supervisor. The form must be taken to Steve Curnock (Acting Safety Officer) on the day of arrival so that he can give the visitor a health and safety induction, please make an appointment with Steve in advance – no work of any kind must be carried out in the department until the visitor has undergone Health and Safety induction.

3.    Key and Proximity card deposit_receipt form (acts as a receipt for refundable £10 deposit). Deposits to be paid to Admin before cards are issued.

N.B. Building access is normally given for a maximum of 12 months and is renewable at the request of the departmental host.

Extension Visitors

No Visitor Application Form is necessary, but the Sponsor must write or email, requesting an extension. Any changes in working practices must be reported to the Safety Office. All Visitors, whether new or not, must sign and return a Visitors Agreement

N.B. Please try to anticipate your Visitor's start date and give as much warning as possible to allow for the formalities to be completed. You should allow a minimum of 6 weeks for this to allow for any visa issues.
